ASBPT Renewal System Unavailable

The Arizona State Board of Physical Therapy provides the ability to renew your license online. Since renewals are only available from July 1 through August 31, 2006 you may not renew your license online at this time. This online service will be made available again for Physical Therapist renewals in 2008.

If you have missed the renewal period and need to submit a reinstatement application, you may download the form from our web-site: www.ptboard.state.az.us. or please call or visit our office.
