ASBP Credential (Permits & Licenses) Applications and Renewals

Welcome to the Arizona State Board of Pharmacy's (ASBP) online licensing application and renewal system. Below you will find a list of permits and licenses (credentials), along with brief descriptions of the requirements and fees associated with them. Click the link below that corresponds to the type of license or permit you would like to apply for or renew.

Note: Renewals are only available between September 15th and November 15th. The length of the renewal period (one year or two) is pre-determined by the credential number.
IF YOU ARE UNABLE TO RENEW YOUR EXISTING LICENSE or PERMIT ONLINE, CALL THE ASBP OFFICE, DO NOT SIMPLY APPLY FOR a NEW LICENSE or PERMIT.

Pharmacy

Application Requirements: New Pharmacies located within the state of Arizona will require floor plans or blueprints of the pharmacy area, a zoning statement or a copy of the lease, and must designate an Arizona licensed pharmacist as the Pharmacist in Charge (PIC). New Pharmacies located outside of Arizona will require a copy of the permit from their state of domicile, and must designate an Arizona licensed pharmacist as the Pharmacist in Charge (PIC).
 
  Fees: New Pharmacy permit fees are prorated based on a $200.00 per year cycle, or a $400.00 per two year cycle, depending upon the permit number issued.
 
Renewal Requirements: The facility's permit number and physical location phone number are required for login verification.
 
  Fees: Existing Pharmacy renewal fees are $200.00 for one year, or $400.00 for two years.


Medical Gas (Distributor or Supplier)

Application Requirements: New Medical Gas Distributors will require a zoning statement or copy of the lease. New Medical Gas Suppliers will require a zoning statement or copy of the lease.
 
  Fees: New Medical Gas Distributor permit fees are prorated based on a $100.00 per year cycle, or a $200.00 per two year cycle, depending upon the permit number issued. New Medical Gas Supplier permit fees are prorated based on a $50.00 per year cycle, or a $100.00 per two year cycle, depending upon the permit number issued.
 
Renewal Requirements: The facility's permit number and physical location phone number are required for login verification.
 
  Fees: Existing Medical Gas Distributor renewal fees are $100.00 for one year, or $200.00 for two years. Existing Medical Gas Supplier renewal fees are $50.00 for one year, or $100.00 for two years.


Manufacturer

Application Requirements: New Manufacturers located within the state of Arizona will require floor plans or blueprints, a zoning statement or copy of the lease, their FDA registration number and listing of drugs stocked, and must designate an Arizona licensed pharmacist as the Pharmacist in Charge (PIC). New Manufacturers located outside of Arizona will require a copy of the permit from their state of domicile, and their FDA registration number and listing of drugs stocked.
 
  Fees: New Manufacturer permit fees are prorated based on a $500.00 per year cycle, or a $1000.00 per two year cycle, depending upon the permit number issued.
 
Renewal Requirements: The facility's permit number and physical location phone number are required for login verification.
 
  Fees: Existing Manufacturer renewal fees are $500.00 for one year, or $1000.00 for two years.


Nonprescription Retailer (Category I or Category II)

Application Requirements: New Nonprescription Retailers will require a list of officers, if the facility is a corporation.
 
  Fees: New Nonprescription Category I permit fees are prorated based on a $50.00 per year cycle, or a $100.00 per two year cycle, depending upon the permit number issued. New Nonprescription Category II permit fees are prorated based on a $100.00 per year cycle, or a $200.00 per two year cycle, depending upon the permit number issued.
 
Renewal Requirements: The facility's permit number and physical location phone number are required for login verification.
 
  Fees: Existing Category I Nonprescription Retailer renewal fees are $50.00 for one year, or $100.00 for two years. Existing Category II Nonprescription Retailer renewal fees are $100.00 for one year, or $200.00 for two years.


Wholesaler (Full Service, Nonprescription, Nonresident Full Service, or Nonresident Nonprescription

Application Requirements: New Full Service Wholesalers located within the state of Arizona will require a $100,000 surety bond or a line of credit from any state, floor plans or blueprints, a zoning statement or copy of the lease, and a resume of the designated representative. New Full Service Wholesalers located outside of Arizona will require a $100,000 surety bond or a line of credit from any state and a copy of the permit from their state of domicile. New Nonprescription Wholesalers located within the state of Arizona will require floor plans or blueprints, a zoning statement or copy of the lease, and a resume of the designated representative. New Nonprescription Wholesalers located outside of Arizona will require a copy of the permit from their state of domicile.
 
  Fees: New Full Service Wholesaler permit fees are prorated based on a $500.00 per year cycle, or a $1000.00 per two year cycle, depending upon the permit number issued. New Nonprescription Wholesaler permit fees are prorated based on a $250.00 per year cycle, or a $500.00 per two year cycle, depending upon the permit number issued.

  Fees: Existing Full Service Wholesaler renewal fees are $500.00 for one year, or $1000.00 for two years. Existing Nonprescription Wholesaler renewal fees are $250 for one year, or $500.00 for two years.


Pharmacist

Application by Score Transfer

Requirements: New Pharmacists will require final transcripts from an approved college of pharmacy, showing the date of graduation.
 
Fees: New Pharmacist application fees are $50 if applying by score transfer, and $300 if applying by full exam or reciprocity. New Pharmacist license fees are prorated based on a $72.50 per year cycle, or a $145.00 per two year cycle, depending upon the license number issued.
 
Renewal Requirements: Your license number and date of birth are required for login verification.
 
  Fees: Existing Pharmacist renewal fees are $72.50 for one year, or $145.00 for two years.


Pharmacy Technician (Certified or Trainee)

Application Requirements: New Certified Pharmacy Technicians will require the PTCB certificate number and expiration date. New Pharmacy Technician Trainees will require a High School diploma or GED, and birth certificate or passport.
 
  Fees: New Certified Pharmacy Technician license fees are prorated based on a $25.00 per year cycle, or a $50.00 per two year cycle, depending upon the license number issued. New Pharmacy Technician Trainee license fees are $35.00 total (includes license and wall certificate) for a two year period.
 
Renewal Requirements: Your license number and date of birth are required for login verification.
 
  Fees: Existing Certified Pharmacy Technician renewal fees are $25.00 for one year, or $50.00 for two years. Existing Pharmacy Technician Trainees do not renew their licenses.


Pharmacy Intern (Student, Foreign, or Graduate)

Application Requirements: New Student Pharmacy Interns will require current transcripts or a letter from the registrar or dean certifying their enrollment and attendance at an approved college of pharmacy. New Foreign Pharmacy Interns will require their original FPGEC certificate. This document will be returned to you; therefore copies will not be accepted. New Graduate Pharmacy Interns will require final transcripts from an approved college of pharmacy, showing the date of graduation.
 
  Fees: New Pharmacy Intern license fees are $60.00 total (includes license and wall certificate) for a 5 year period.
 
Renewal Requirements: Existing Pharmacy Interns do not renew their licenses.
 
  Fees: Existing Pharmacy Interns do not renew their licenses.
